Comprehensive Care of Patients with Spastic Paresis – A Long-Term Commitment

چکیده
Spastic paresis is a frequent complication of lesions to central motor pathways. Patients who develop spasticity require specialised rehabilitation programmes to alleviate their symptoms, help them to relearn motor skills and regain some independence. Long-term management of patients with spastic paresis requires a comprehensive approach that addresses the patient’s physical needs and mental wellbeing, and continues throughout all stages of the rehabilitation process. There are three pillars of success for long-term rehabilitation programmes in spastic paresis. Since most human behaviour is goal-directed, the first is to employ patient-centred goal setting to engage and motivate patients to play an active part in their own rehabilitation. Goals should be meaningful for and achievable by the patient, and revised as necessary. The second foundation of success is to ensure detailed clinical assessment, using appropriate spasticity assessment scales to inform Botulinum toxin treatment plans -doses and muscles to be injected. Lastly, the use of guided self-rehabilitation programmes encourages an active role for patients in their treatment and maximises outcomes. This paper summarises proceedings from the Comprehensive Care of Patients with Spastic Paresis: A Long-Term Commitment symposium, at the International Society of Physical and Rehabilitation Medicine World Congress, 2015.

Globally, aging populations are driving the demand for long-term care (LTC) services for a growing number of older people with disabilities or chronic illnesses. A key challenge for policy-makers in all countries is to find a comprehensive solution to financing LTC services to make them widely accessible, affordable, and equitable for all in need. In this commentary, we...
متن کاملCompletely Intracorporeal Robotic-Assisted Laparoscopic Ileovesicostomy
We present a report of a completely intracorporeal robotic-assisted laparoscopic ileovesicostomy with long term follow-up. The patient was a 55-year-old man with paraplegia secondary to tropical spastic paresis resulting neurogenic bladder dysfunction. The procedure was performed using a da Vinci Surgical system (Intuitive Surgical, Sunnyvale, CA) and took 330 minutes with an estimated blood lo...
متن کاملNeural Networks in Electric Load Forecasting:A Comprehensive Survey
Review and classification of electric load forecasting (LF) techniques based on artificial neuralnetworks (ANN) is presented. A basic ANNs architectures used in LF reviewed. A wide range of ANNoriented applications for forecasting are given in the literature. These are classified into five groups:(1) ANNs in short-term LF, (2) ANNs in mid-term LF, (3) ANNs in long-term LF, (4) Hybrid ANNs inLF,...
متن کاملSpastic movement disorder: impaired reflex function and altered muscle mechanics.
In clinical practice, signs of exaggerated tendon tap reflexes associated with muscle hypertonia are generally thought to be responsible for spastic movement disorders. Most antispastic treatments are, therefore, directed at the reduction of reflex activity. In recent years, however, researchers have noticed a discrepancy between spasticity as measured in the clinic and functional spastic movem...
